Brisbane Water Fluoridation: What It Means for Your Teeth

How Brisbane's fluoridated water supply helps prevent tooth decay, what the evidence says, and what to know as a Brisbane resident.

Brisbane has had fluoridated water since December 2008. For residents across the city, from Everton Park to Nundah and everywhere in between, this means the tap water you drink and cook with contains a small amount of fluoride designed to help protect teeth from decay.

Water fluoridation is one of the most studied public health interventions in history. It is also one of the most debated online. Here is what the evidence says and what it means for your dental health.

What Is Water Fluoridation?

Water fluoridation is the controlled addition of fluoride to a public water supply. The goal is to raise the fluoride concentration to a level that reduces tooth decay across the population.

In Brisbane, the fluoride level is maintained at approximately 0.7 parts per million (ppm). This is the level recommended by the Australian National Health and Medical Research Council (NHMRC). It is low enough to be safe for daily consumption across all age groups, and high enough to provide a measurable reduction in tooth decay.

Fluoride occurs naturally in water sources at varying concentrations. In some regions of the world, natural fluoride levels exceed the recommended range. In Brisbane, natural levels are below the target, so fluoride is added during water treatment.

How Fluoride Protects Teeth

Fluoride works through two primary mechanisms:

Remineralisation. Throughout the day, acids produced by bacteria in the mouth dissolve minerals from the enamel surface (demineralisation). Fluoride in saliva promotes the redeposition of calcium and phosphate back into the enamel (remineralisation). This repairs early damage before it becomes a cavity.

Enamel resistance. When fluoride is incorporated into the enamel structure during remineralisation, it forms fluorapatite, which is more resistant to acid attack than the original hydroxyapatite. This means the enamel surface becomes harder to dissolve with each remineralisation cycle.

These effects are topical, meaning they happen at the tooth surface. Drinking fluoridated water bathes the teeth in a low concentration of fluoride throughout the day, providing continuous low-level protection.

What the Evidence Shows

The evidence base for water fluoridation is extensive. Key findings include:

The NHMRC review (2017) concluded that water fluoridation at 0.6 to 1.1 ppm reduces tooth decay by 26 to 44 percent in children and adolescents, and by around 27 percent in adults. The review found no reliable evidence of adverse health effects at recommended levels.

The Cochrane review (2015) found that introducing water fluoridation reduced decay in children's permanent teeth by an average of 1.3 fewer decayed, missing, or filled tooth surfaces. The review noted limitations in the evidence quality but confirmed a consistent direction of benefit.

Population-level data from Queensland provides a natural experiment. Before 2008, Queensland had among the highest rates of childhood tooth decay in Australia, partly because most of the state's water was not fluoridated. Since the introduction of fluoridation, studies have documented measurable improvements in dental health outcomes, particularly in children.

The Australian Dental Association, the World Health Organization, and dental associations in the United States, United Kingdom, Canada, and New Zealand all endorse community water fluoridation as a safe and effective public health measure.

Common Concerns Addressed

Despite the strength of the evidence, water fluoridation generates questions. Here are the most common.

Is fluoride safe at the levels in Brisbane water? At 0.7 ppm, the fluoride concentration in Brisbane water is well below the level at which any adverse effects have been documented. The NHMRC has reviewed the safety evidence multiple times and consistently concluded that water fluoridation at recommended levels does not pose a health risk. The margin of safety between the level used in water fluoridation and the level at which health effects might occur is substantial.

What about dental fluorosis? Dental fluorosis is a cosmetic change in the appearance of enamel caused by excessive fluoride intake during tooth development (typically in children under eight). At the levels used in Brisbane water, the risk of fluorosis is low, and any fluorosis that does occur is typically mild (faint white lines on the teeth that are often not visible to the untrained eye). Moderate to severe fluorosis is associated with fluoride intakes well above what community water fluoridation provides. To reduce fluorosis risk in young children, use a small amount of low-fluoride toothpaste and supervise brushing to prevent swallowing.

What if I use a water filter? Standard carbon filters (such as Brita jugs) do not remove fluoride from water. Reverse osmosis systems and distillation do remove fluoride. If your household uses reverse osmosis filtration, you may not be receiving the dental benefit of fluoridated water. Discuss this with your dentist at your next check-up.

What about bottled water? Most bottled water sold in Australia contains very low levels of fluoride, often below 0.1 ppm. If bottled water is your primary drinking water, you are likely not receiving meaningful fluoride exposure from water. This is relevant for children during tooth development and for adults with elevated decay risk.

Fluoride and Children in Brisbane

Children benefit most from water fluoridation because their teeth are still developing and their enamel is more susceptible to decay.

For Brisbane families, the practical implications are:

  • Encourage tap water. When children drink fluoridated tap water rather than juice, soft drinks, or unfluoridated bottled water, they receive both the hydration benefit and the fluoride benefit.
  • Continue using fluoride toothpaste. Water fluoridation supplements but does not replace the need for fluoride toothpaste. The concentration in toothpaste (1,000+ ppm) is much higher than in water (0.7 ppm), and the direct contact with tooth surfaces during brushing provides additional protection.
  • Attend regular dental visits. Children aged 0 to 17 may be eligible for the Child Dental Benefits Schedule (CDBS), which provides up to $1,158 in covered dental services over a two-year period.   • Supervise brushing. For children under six, use a pea-sized amount of low-fluoride toothpaste and ensure they spit rather than swallow. This minimises fluoride ingestion while maintaining the topical benefit.

Fluoride Is One Part of Prevention

Water fluoridation is a population-level measure. It reduces decay rates across a community, but it does not eliminate the need for individual preventive care.

You still need to:

  • Brush twice daily with fluoride toothpaste
  • Floss daily
  • Limit sugary foods and acidic drinks
  • Attend regular dental check-ups and professional cleans
  • Address any specific risk factors (dry mouth, high sugar diet, orthodontic appliances) with your dentist

Most bottled water in Australia does not contain fluoride at levels that provide dental benefit. If bottled water or filtered water (particularly reverse osmosis filtered) is your primary drinking water, you may be missing the protective benefit of community fluoridation. Discuss this with your dentist.
