Which Toothpaste Should You Use? A Dentist's Guide

How to choose the right toothpaste for your teeth. A Brisbane dentist explains fluoride, sensitivity, whitening, and what the labels mean.

The toothpaste aisle is overwhelming. Whitening, sensitivity, charcoal, natural, fluoride-free, prescription-strength. Every box promises something different. Most patients at our Brisbane clinics admit they just grab whatever is on sale.

That approach is fine for some people. But toothpaste is not one-size-fits-all, and the wrong choice can leave you underprotected or paying for ingredients you do not need. Here is how to make a more informed decision.

The One Ingredient That Matters Most

Fluoride. Everything else is secondary.

Fluoride strengthens tooth enamel by promoting remineralisation. It reduces the rate at which acids from food and bacteria dissolve the enamel surface. It also makes enamel more resistant to future acid attacks.

A fluoride toothpaste with at least 1,000 ppm (parts per million) is the baseline recommendation from the Australian Dental Association and the World Health Organization. Most adult toothpastes in Australia contain between 1,000 and 1,500 ppm. This range is effective and safe for daily use.

If you pick a toothpaste with adequate fluoride and brush for two minutes twice a day, you are covering the most important base. Everything else is refinement.

Toothpaste for Sensitive Teeth

If cold water, hot coffee, or sweet foods trigger a sharp twinge, you may benefit from a desensitising toothpaste. The two most common active ingredients for sensitivity are:

Potassium nitrate. Found in many Sensodyne and Colgate Sensitive products. It works by calming the nerve inside the tooth, reducing its response to stimuli. It takes about two weeks of regular use to reach full effect.

Stannous fluoride. Found in some Oral-B and Sensodyne products. It works differently, forming a protective layer over exposed dentine that blocks the tubules (tiny channels) through which stimuli reach the nerve. It also has antibacterial properties that may benefit gum health.

If sensitivity is severe or sudden, book a check-up rather than relying on toothpaste alone. Sensitivity can signal decay, a cracked tooth, or gum recession that needs clinical attention.

Whitening Toothpaste: What It Can and Cannot Do

Whitening toothpaste works primarily through mild abrasives that remove surface staining. Some also contain low concentrations of hydrogen peroxide or blue covarine, an optical agent that makes teeth appear less yellow temporarily.

What whitening toothpaste can do:

  • Remove light surface staining from tea, coffee, and red wine
  • Help maintain results after professional whitening
  • Make teeth appear marginally brighter with regular use

What whitening toothpaste cannot do:

  • Change the intrinsic colour of your teeth
  • Match the results of professional whitening
  • Reverse yellowing caused by ageing, medication, or thin enamel

If you are choosing a whitening toothpaste, check the RDA (Relative Dentin Abrasivity) score. Anything under 100 is considered safe for daily use. Toothpaste for Gum Health

If your dentist has flagged early gum disease or you notice bleeding when brushing, a toothpaste formulated for gum health may help as part of a broader treatment plan.

Look for:

  • Stannous fluoride. Reduces bacteria associated with gingivitis and forms a protective barrier on tooth and gum surfaces.
  • Sodium bicarbonate (baking soda). Mildly abrasive with some evidence of plaque reduction. Found in some Arm & Hammer products.
  • Antibacterial agents. Some prescription toothpastes contain chlorhexidine, though these are not suitable for long-term daily use due to staining.

Toothpaste alone will not treat established gum disease. Professional scaling and improved flossing technique are far more impactful. But the right paste can support your daily routine while you work with your dentist on a treatment plan.

High-Fluoride Toothpaste

For patients with a high risk of decay, a prescription-strength toothpaste containing 5,000 ppm fluoride may be recommended. This is significantly stronger than standard toothpaste and is available only through a dentist.

High-fluoride toothpaste is commonly prescribed for:

  • Patients undergoing orthodontic treatment (braces or aligners increase decay risk in some cases)
  • Patients with dry mouth caused by medication or medical conditions
  • Patients with a history of frequent cavities
  • Older adults with root surface exposure

If your dentist at Ivy Dental prescribes a high-fluoride toothpaste, use it as directed. It is not a substitute for regular brushing and flossing, but it adds a meaningful layer of protection for high-risk patients.

Children's Toothpaste

Toothpaste recommendations vary by age:

  • Under 18 months: Clean teeth with a soft cloth or brush and water only.
  • 18 months to 6 years: Use a pea-sized amount of low-fluoride toothpaste (around 500 ppm), or a smear of standard toothpaste for children at higher risk of decay. Your dentist can advise which is appropriate.
  • 6 years and older: Standard adult fluoride toothpaste (1,000+ ppm) is suitable.

Children eligible for the Child Dental Benefits Schedule (CDBS), which covers up to $1,158 in dental services for children aged 0 to 17, can have their toothpaste needs discussed at a bulk-billed check-up with $0 out of pocket while the remaining Medicare balance covers the planned items.

What About Fluoride-Free Toothpaste?

Fluoride-free toothpastes are marketed as natural alternatives. They typically rely on ingredients such as xylitol, baking soda, or essential oils for cleaning.

While xylitol has some evidence for reducing cavity-causing bacteria, and baking soda is a mild abrasive, neither provides the enamel-strengthening effect of fluoride. For patients with low decay risk and excellent hygiene, a fluoride-free paste may not cause immediate problems. But it removes a layer of protection that is easy to maintain and hard to replace.

Our recommendation: unless you have a specific medical reason to avoid fluoride (which is rare), use a fluoride toothpaste. The evidence is clear and consistent.

How to Read the Label

When comparing toothpaste products, look for:

  • Fluoride concentration. Listed in ppm. At least 1,000 ppm for adults.
  • Active ingredients. These are the ingredients with clinical effects: fluoride, potassium nitrate, stannous fluoride, hydrogen peroxide.
  • RDA score. Not always listed on packaging, but available from manufacturers or dental associations. Lower is gentler.
  • ADA endorsement. Products carrying the Australian Dental Association seal have been independently assessed.

Yes. Fluoride toothpaste at standard concentrations (1,000 to 1,500 ppm) is safe for daily use in adults and children over six years of age. Decades of research and endorsement from every major dental and health authority worldwide support its safety and effectiveness in preventing tooth decay.

What toothpaste do dentists recommend?

Most dentists recommend a fluoride toothpaste with at least 1,000 ppm fluoride and a low to moderate abrasivity rating. Beyond that, the choice depends on your individual needs. Patients with sensitivity, gum disease, or high decay risk may benefit from specific active ingredients.

Is natural toothpaste as effective as regular toothpaste?

It depends on the formulation. A natural toothpaste that contains fluoride at the recommended concentration can be effective. However, many natural toothpastes marketed as fluoride-free do not provide the same level of cavity protection. Check the active ingredients rather than relying on the brand positioning.

Should children use adult toothpaste?

Children under six should use a low-fluoride toothpaste (around 500 ppm) or a smear of standard-strength toothpaste, depending on their caries risk. From age six, standard adult fluoride toothpaste is appropriate. Your dentist can advise based on your child's specific risk factors.
