Sugar and Your Teeth: How Much Is Too Much?

How sugar causes tooth decay, how much is safe, and practical ways to reduce the damage without cutting sugar entirely.

Sugar is the most well-established dietary cause of tooth decay. That is not new information. But the relationship between sugar and your teeth is more nuanced than "sugar is bad." The type of sugar, how often you eat it, and what you do afterward all affect the outcome.

Here is what happens when sugar meets your teeth, what the research says about safe limits, and practical ways to reduce the damage without unrealistic dietary restrictions.

How sugar causes decay

Sugar itself does not damage teeth. Bacteria do. Here is the process:

  1. You eat or drink something containing sugar.
  2. Bacteria in your mouth (particularly Streptococcus mutans) feed on the sugar.
  3. As the bacteria metabolise the sugar, they produce acid as a byproduct.
  4. That acid dissolves the mineral content of tooth enamel in a process called demineralisation.
  5. Saliva works to neutralise the acid and redeposit minerals (remineralisation), but this takes time.
  6. If acid attacks happen faster than your saliva can repair the damage, the enamel breaks down and a cavity forms.

The key insight is that decay is driven by the frequency and duration of acid attacks, not just the total amount of sugar consumed. Sipping a sugary drink over two hours produces a sustained acid attack. Drinking the same amount in five minutes produces a shorter one.

Frequency matters more than quantity

A Brisbane patient who drinks one can of soft drink with lunch will experience one acid attack lasting roughly 20 to 30 minutes. A patient who sips from the same can over three hours will have a continuous acid attack for the entire period.

The same principle applies to snacking. Three meals a day means three acid exposure events. Adding four snacks between meals (biscuits, muesli bars, dried fruit, flavoured yoghurt) adds four more acid events. The teeth get less recovery time between attacks.

This is why dentists tend to focus on patterns rather than specific foods. The occasional slice of cake at a birthday party is not a concern. Constant grazing on sweet snacks throughout the day is.

Types of sugar and dental risk

Not all sugars behave the same way in the mouth.

Free sugars. These are the sugars added to foods and drinks (table sugar, honey, syrups, fruit juice concentrates) as well as sugars naturally present in fruit juices, smoothies, and honey. Free sugars are the primary dietary cause of decay.

Intrinsic sugars. These are sugars contained within whole fruit and vegetables. Because they are locked within the cell structure of the food, they are released more slowly and are less available to oral bacteria. Whole fruit poses a lower decay risk than fruit juice.

Milk sugars (lactose). Lactose is less readily fermented by decay-causing bacteria than sucrose. Plain milk is considered low-risk for dental decay. Flavoured milk, however, contains added free sugars.

Starches. Refined starches (white bread, chips, crackers) can be broken down into sugars by enzymes in saliva. On their own, they are less cariogenic than free sugars, but when combined with sugar (e.g. biscuits, cakes), they increase the sticky, retentive nature of the food.

The WHO recommendation

The World Health Organization recommends limiting free sugar intake to less than 10% of total daily energy intake, with additional health benefits at less than 5%. For an average adult, 5% equates to roughly 25 grams or 6 teaspoons per day.

To put that in context:

  • A 375ml can of cola contains approximately 40 grams of sugar (10 teaspoons)
  • A 600ml bottle of flavoured iced tea contains approximately 30 to 40 grams
  • A standard muesli bar contains approximately 8 to 12 grams
  • A tablespoon of tomato sauce contains approximately 4 grams

Many Australians exceed the 10% threshold without realising it, particularly through drinks.

Practical ways to reduce sugar damage

Eliminating sugar entirely is unrealistic for most people. These strategies reduce the dental impact without requiring a complete dietary overhaul.

Consolidate sugar intake to mealtimes

If you are going to eat something sweet, have it with or immediately after a meal rather than as a standalone snack. Saliva production is higher during meals, which helps neutralise acid faster. The food itself can also help buffer the acid.

Reduce sugary drinks or change how you drink them

Sugary and acidic drinks are the biggest single contributor to acid attacks. If you drink them, finish the drink in a sitting rather than sipping over a long period. Better still, swap to water for regular hydration. Brisbane tap water contains fluoride, which provides a mild protective benefit to teeth throughout the day.

Rinse with water after sugar exposure

Swishing water around your mouth after eating or drinking something sweet helps dilute the acid and clear residual sugar. It is not as effective as brushing, but it is a practical step when brushing is not possible (e.g. at work or school).

Wait 30 minutes before brushing after acidic food

This sounds counterintuitive, but brushing immediately after an acid attack can remove softened enamel. Waiting 30 minutes allows saliva to begin remineralising the surface before you brush. Rinsing with water in the interim is fine.

Use fluoride toothpaste

Fluoride strengthens enamel and enhances remineralisation. Brushing twice daily with a fluoride toothpaste is one of the most effective measures against decay. For patients at higher risk, a fluoride rinse used at a different time to brushing can provide additional protection.

Check labels for hidden sugars

Sugar appears under many names on food labels: sucrose, glucose, fructose, maltose, dextrose, corn syrup, rice malt syrup, agave, and others. Products marketed as "healthy" (granola, smoothie bowls, protein bars) often contain significant free sugars.

Children and sugar

Children's teeth are more susceptible to decay because their enamel is thinner and less mineralised than adult enamel. For children, the same principles apply with even more importance:

  • Limit juice and cordial. Water and plain milk are the right drink choices.
  • Avoid prolonged bottle feeding with milk or juice at bedtime.
  • Introduce tooth brushing as soon as the first tooth appears.
  • Make sugary treats occasional, not daily.

When to see your dentist

If you notice white spots on your teeth (early demineralisation), sensitivity to sweet foods, or visible discolouration, book a checkup. Early decay can sometimes be managed without a filling if it is caught at the demineralisation stage. Once a cavity has formed, a dental filling or other restoration is needed.
